Camp Refund & Release Policy
As the parent or legal guardian of named Participant. In consideration for Participant’s participation in a program being held by STEM Revolution, I, as Participant’s parent or legal guardian, on behalf of Participant, his/her heirs, executors, and administrators, hereby voluntarily, irrevocably, and unconditionally release and hold harmless STEM Revolution, their respective trustees, officers, employees, agents and assigns (hereinafter ”Releasees”), from any and all manner of claims, actions or causes of actions, whether related to or arising from the negligence of Releasees or otherwise, arising from or in connection with Participant’s participation in the Program, including but not limited to arising from Participant’s presence on the premises of leased locations and STEM Revolution or the Participant being transported to or from or traveling to or from the premises of leased locations and STEM Revolution. This release includes but is not limited to claims of active or passive negligence, products liability, personal injury, death or damage to property or violation of any laws or regulations
Consent To Treat
I (we) the undersigned participant, parent or legal guardian of the registered person hereby authorize treatment of said participant by a licensed medical physician in case of any accident or illness that may arise, or any hospitalization necessary, IF parent can not be reached.
I grant to STEM Revolution, its representatives and employees the right to take photographs of my child and to use and publish the same in print and/or electronically. I agree that such photographs of my child without my child’s name can be used for purposes including publicity, illustration, advertising, and Web content.
Code Of Conduct
Expectations 1. Follow the camp director’s and coaches’ directions. 2. Always stay with the group. 3. Be courteous to others. Remember to treat others as you would like to be treated. Verbal abuse and inappropriate physical contact towards another camper will not be tolerated. 4. Always talk to the coach if you have a question or a problem during camp. We won’t be able to help you if you don’t speak up. 5. Pay attention and listen when someone else is speaking. Failure to follow these expectations will result in an unsafe or unenjoyable atmosphere for you and others in camp. Should that happen, the following consequences will occur: Consequences Depending on the severity of the situation or if the behavior persists, one or more of the following consequences will be taken. 1. Coaches will discuss the behavior with the camper. 2. The camper will be given a ”time out” or will not be allowed to participate in the subsequent activity. 3. Coaches will discuss the behavior with the parent/guardian. 4. The child will be sent home immediately or told that he/she will not be allowed to return the following day.
Full refund up to the three weeks prior to the start of camp minus a nonrefundable $50 registration fee. After the three weeks prior to camps, you may receive a 50% refund. No refunds accepted after the 7 days prior to the start of camp.
